INFORMATION REQUIRED

·          Ohio Driver's License or Identification Card. If you do not have a driver's license, a Birth Certificate, Social Security Card, or Passport is required.

·          Address for agency to receive fingerprint/background check certificate.

·          Cash only payment to be made upon completion

·          If the person being fingerprinted is a minor, a parent or guardian must accompany the minor to sign additional paperwork giving our office permission to fingerprint.

·          If the agency you are being printed for is to be billed, please note that information in the COMMENTS section
